Add root ChangeLog.md index for multi-package repo

Smartypants requested to merge add-root-changelog into master

Adds a root ChangeLog.md that links to per-package changelogs (horizon-spec/ and horizon-spec-tests/). Satisfies policy §2.1 root ChangeLog.md requirement for multi-package repos per @nastypants QC audit and @locallycompact's direction (option 1: root index file).
